1. Acknowledgment of Risks
I, the undersigned participant, acknowledge that participation in fitness classes and activities at
PowerZone Studio LLC, including but not limited to strength training, cardio exercises, functional
training, trampoline workouts (Jump), Booty Sculpt classes, group fitness, personal training, use of
equipment, and all other physical activities, involves inherent risks.
These risks include, but are not limited to:
• Muscle strains, sprains, or tears
• Joint injuries
• Falls, slips, or loss of balance
• Impact injuries from equipment or other participants
• Cardiovascular complications
• Dizziness, fainting, or dehydration
• Accidents occurring anywhere in the facility, including the Kids Area
I voluntarily choose to participate and assume all risks, known or unknown, associated with these
activities.
2. Health Declaration
I certify that:
• I am physically able to participate in fitness activities
• I have no medical condition that would prevent safe participation
• I have consulted a physician if I have any concerns
• I will inform PowerZone staff of any medical conditions that may affect my safety
I agree to stop immediately if I feel pain, dizziness, or discomfort and seek medical attention.
3. Assumption of Full Responsibility
I understand and agree that I am solely and fully responsible for:
• Monitoring my own physical condition
• Using equipment properly and safely
• Following all instructions from staff
• Exercising within my personal limits
I acknowledge that any guidance provided does not replace my responsibility for my own safety.
4. Release of Liability
In consideration of being allowed to participate, I hereby waive, release, and forever discharge
PowerZone Studio LLC, its owners, managers, trainers, employees, contractors, agents, and
representatives from any and all claims arising from:
• Injuries sustained during participation
• Accidents occurring on the premises
• Equipment use
• Actions of other participants
• Ordinary negligence of PowerZone or its staff
This release does not apply to gross negligence or intentional misconduct as defined under Florida law.
5. Equipment Use & Facility Rules
I agree that:
• Equipment must be used properly
• Facility rules must be followed at all times
• PowerZone may deny or terminate participation for unsafe behavior
• PowerZone is not responsible for lost, stolen, or damaged personal items
6. Kids Area Waiver
If I use the Kids Area, I acknowledge that:
• It is not a babysitting service
• Supervision is limited to posted hours
• I remain solely and fully responsible for my child/children at all times
• Children must stay in the designated area
• Children may not use gym equipment
• PowerZone is not liable for injuries or incidents involving children
7. Photography & Media Release
Please select one:
( ) I authorize PowerZone to use my image and likeness in photos/videos for marketing and social
media.
( ) I do NOT authorize the use of my image.
This choice does not affect my membership or participation rights.
8. COVID-19 / Infectious Disease Waiver
I acknowledge that exposure to contagious diseases may occur and agree that PowerZone:
• Cannot guarantee I will not be exposed
• Follows reasonable safety measures
• Is not liable for illness resulting from participation
I participate at my own risk.
9. Binding Agreement
This waiver:
• Is legally binding
• Applies to my heirs, executors, successors, and assigns
• Is governed by the laws of the State of Florida
10. Certification of Understanding
By signing, I confirm that:
• I have read and fully understand this waiver
• I am signing voluntarily and without coercion
• I understand I am giving up substantial legal rights
